The litmus test for me is if you have a recent backup and you do a
bpimmedia -mediaid ###### and you get a no entity returned, you are
probably ok to deassign the tape.

If its not in the images database, you should be okay to deassign the
tape in the volume and/or media dbase.  Unless of course you think
something real fishy is going on, then try the import, which you did and
it still failed, so you have a case where Netbackup is not deassigning
your media...

vmquery -deassignbyid   sounds like the answer to your problem.

I bet you had a backup running and it got through tape 1, went to tape
2, succeeded and on tape 3, 4, or 5 it failed.

So there is no image info on the tape, nothing in media DB, but the tape
1 and or 2 still shows assigned by the volume database.

I think this happens from time to time and Veritas doesn't have a good
handle on it, meaning NetBackup doesn't know how to clean up that
situation.  Its one reason I created my own consistency check script
similar to the one they will run for you if you call them. Its supposed
to be better then the one you can run on your own (bpdmb -consistency 2
or something like that)

I suspect this because the support staff has admitted they know there
are some funky issues that crop up while trying to keep 3 databases in
sync, and of course they say these will be addressed in the next
release.  Just my own opinion, may not be worth a grain of salt.
