We had something similar about three weeks ago that affected only one of our
three file servers.  We are in an NT4 domain.  We applied the
NB_341_3A.winnt.intel_247505.exe patch and that cleared things up for us.

We had this exact same issue last year and have worked painfully with
Veritas for many months to get this problem resolved.

I can say, that after more than 12 months as a critical issue within
Veritas, that we are no closer to a fix than you are right now.  However, as
a work around, we found that replacing the file list directive
ALL_LOCAL_DRIVES with / will cause the backups to get more than just the C:
or D: identifier and actually back up files and directories on the disks.

This all comes down to OTM and how it returns drive information during the
start of a backup.  Another work around would be disabling OTM.
