First of all, this is going to change in 4.5.  These files are not
really meant to be parsed manually, bpdbjobs is a better alternative.

the big number is a unix time (sec since 01/01/70).  FW is files written.

KB written, along with a time and a KB/s.

No, not nearly every file.  Something like very 500 files or 30,000KB
it'll update (possibly every 25 until 250, then 500's).

bpflist, once the backup is done.  

netbackup/db/images/$CLIENT/$TIME/*.f  (though this will probably
change in 4.5 if you use the new format).

On the client, you could enable verbose bpbkar logging.  See the
troubleshooting guide for error 41 and the step which talks about
bpbkar_path_tr.   BTW, you could also use 

  bperror -r -S 41
