We had the same problems. After much trial and error with Veritas tech
support, we renamed the /usr/openv/netbackup/db/failure_history so it would
start over with new history and the backups started running beautifully and
have kept doing so.
In a Windows installation, this file is located at install_path:\program
files\veritas\netbackup\db.
