Gina:

I have a client who has experienced that very thing this past weekend.  They 
have a Powderhorn Library running with ACS/LS.  From what I gather from the 
client, it happened on a media server, and nothing is in the logs showing any 
daemon activity.

I'm trying to see if we can replicate now...not likely.
