What drives are you using ?

Are you running with the latest/recommended firmware of the drives ?

What brand are the cartridges ?

What library are you using ?

Do you see any kernel errors ?

I usually put the following line into my /etc/syslog.conf so I have all
kernel related issues in a single file:

kern.debug /var/adm/kernel

(and don't forget to kill -HUP the syslogd).

Can you send the output of the inquire command ?
