So the tape is labeled 'Monthly029', but there's no such volume in the
database.

Can you confirm that label with scanner?  (scanner -inv -f <drive>).
The beginning of it will spit out the label.  The -n will keep it from
modifying any of the databases.  ^C to exit.  Scanner won't have
anything to do with the barcode, so if it says 'Monthly029', it's
because the tape has that written to it, not because it's has some
similarity to the barcode.

If that tape really is Monthly029, and it's not in the database, the
question is how it got that name (when was it labeled?).  Is it an old
tape with a new barcode?
