Ouch! 6.0 (aprt from being very old) was known for a serious media
database corruption bug. It was replaced very quickly by 6.0.1 which
fixed the bug. This is very likely the cause of your problem. Before you
do anything else you should upgrade your server software. You can go to
6.1.4 without any upgrade enablers.
