We solved a similar problem doing the following:

The following patches had to be installed. They all relate to Sun Alert
57516 that JUST came out.

First off, the machine had to be up-revved to kernel patch 117000-05.
After that the following patches had to be installed:

108528-29, 108725-15. Then install 116962-02, 116975-01 and 109896-25. The
last three are actually the patches that fix the problem. The rest of them
were pre-requisites to getting the machine to a level to install the
necessary patches.
