3 months ago we got a Qualstar 5466 and had some issues with 39160 cards
that might be similar to what you are talking about.

Setup:
Linux storage node, Legato 7.1, Fedora Core 1, 2.4.22-2.4.26 kernel, old
and new adaptec driver, Proliant DL380G2, HPaq Gbit nic, HPaq relabeled
39160 dual port, new scsi cables and terminators. We had and tried extra
parts of above.

Whatever kernel, driver and mix of parts, I would sooner (sometimes on
bootup) or later see scsi errors on the proliant. Those errors were
always on the 39160 scis buses used for the library. The tapedrive would
stop responding to the host computer until both were rebooted.

I went to a dual port LSI 22320 and stopped the scsi errors entirely.
For some reason (linux drivers?) I had to put the robot on a separate
scsi port. I used the proliant external raid port, have not had any
issues since. I did find many google references to problems with Linux
and adaptec 160/320 cards. LSI was widely recommended.

This may or not have anything to do with your errors.
My descripton of our hardware and software were more along the lines of
what Terry would like to read I think.
