The others I'm not as certain of. This one's pretty easy. The
bootstrap information is read in as the media database. When the
bootstrap was saved, the bootstrap wasn't complete, so it's not visible
in the media database.
Whenever I do a mmrecov, I always follow it up by scanning the tape from
the bootstrap forward.

Of course you can. The bootstrap is there, but the recovered media
database is from a point just before it was created. You could retry
the recovery immediately. Or, you can use 'scanner' to bring the
knowledge of the bootstrap into the database.
