I am still having tape problems so here is the detail about my system.

Legato has had me add the following to the st.conf:

tape-config-list=

"IBM     ULTRIUM-TD1", "IBM Ultrium", "CLASS_3580",
"IBM     ULT3580-TD1", "IBM 3580 Ultrium", "CLASS_3580";
CLASS_3580 = 1, 0x24, 0, 0x45863d, 2, 0x00, 0x01, 0;

I then did a boot -r.  

I am running (uname -srv) SunOS 5.9 Generic_112233-04

I just added patch 13277-19.  Here is a complete list of patches from
patchadd -p:

Do you have an explicit entry for LTO in the st.conf, or are you using
the built-in settings?

What OS and st patches do you have?
