How do you test that this is the problem?

Difficult to know. The error message about block size (read 32768
bytes...) is often a good clue. If you find tapes being prematurely
marked full at random capacities this can be an indication too, although
this is not the only case that will cause these symptoms.

A process of elimination can help you to track this down. Connect only
the bare minimum together to try to trace the cause of your problems.

If this is the problem, how can it be resolved?
One thing is to zone your SAN as much as possible. You reduce the
possibility of totally unrelated machines doing unexpected things.
The rest of it depends on your kit. Some SAN equipment has settings that
control whether they send SCSI resets or not. Some kit can filter out
SCSI resets from hosts on the SAN.
