This is looking more and more like a hardware issue. I'm working with

(Exabyte) support to see if it is something I can fix or if the drive needs to

replaced. The suggestions about drive cleaning and bad tapes were right on the
money, except that cleaning the drive and replacing the media don't seem to fix
it.
