I had this problem but it was not random.
My problem turned out to be that in the .../client/api/bin/dsm.sys, the 
PASSWORDDIR path was not correct.

I am randomly seeing these message on all of my 6.x (6.1.5.10 and
6.2.3.100) servers and have no clue why.

1/30/2012 12:00:18 PM ANR1893E Process 1304 for Database Backup completed with 
a completion state of FAILURE.
