did you see actlog file...
some times this happened if tsm tries to access half filled bad tape of same 
node.Try to trace Ur act logfile when this problem happens.Take that error code 
and get reasons. If necessary check tape device class matching on that 
particular storage pool.
and also check which tapes tsm trying to access when issued back
(tsm> q mount) check that volume if anything wrong.Some times it works after 
making that volume unavailable.
