Hi Arnaud!
I have encountered this too in the past and it also took me quite some time
to figure it out. My solution was to delete the scsi device which is
emulated over the fiber channel connection.
Just issue the command rmdev -dl fscsi0 and run cfgmgr, this will probably
solve your problem too.

I got a strange problem 2 days ago, and still no explanation : the path
to an LTO drive located in a 3584 library was suddenly reported as
marked offline :
12/02/03 07:51:22 ANR8471E Server no longer polling drive LTO1DR4 in
library LTOLIB1 - path /dev/rmt4 will be marked off-line.
12/02/03 07:51:22 ANR8873E The path from source ADSM-PAC to destination
LTO1DR4 (/dev/rmt4) is taken offline.
I had no chance to have it back on line again, despite the fact the
drive was looking OK on AIX side. I then called IBM, that supposed
something was wrong with the drive and replaced it : still no success
Trying to delete and recreate the path gives :
12/03/03 08:47:55 ANR2017I Administrator BAR021 issued command: DEFINE
PATH adsm-pac lto1dr4 srctype=server desttype=drive device=rmt4
library=ltolib1
12/03/03 08:47:55 ANR8779E Unable to open drive rmt4, error number=2.
This drive is SCSI type, connected to a 2108 G07 san data gateway, which
detects  the drive (we actually have 2 scsi connectors on the box, each
one having two LTO drives):

Today I recycled all the hardware : server, 3584, SAN gateway, thinking
it could correct an eventual network error, but still got the problem:

On TSM server initialization :

12/05/03 09:18:13 ANR8470W Initialization failure on drive LTO1DR4 in
library LTOLIB1.

On attempt to redefine the path :
12/05/03 09:29:42 ANR2017I Administrator BAR021 issued command: DEFINE
PATH adsm-pac lto1dr4 srctype=server desttype=drive device=rmt4
library=ltolib1
12/05/03 09:29:42 ANR8420E DEFINE PATH: An I/O error occurred while
accessing drive LTO1DR4.
I'm really lost, has anyone an idea of what could be happening here ?
