I did power down everything, re-check all the scsi connections,
clean drives again, and go from 'scratch' (roughly 3 hours to
do it). Still got the same errors.
I can run dsmserv restore db todate=today
and it loads the tape, before getting the error.
I am about to swap the two drives we have, (and their scsi targets)
and retry. ... Wish me luck.
Any other suggestions are appreciated.

Yes, sorry. Yes, Windows 2K server, 4.2.3.1 server software.
library is SCSI attached 3583 with the same microcode we have
on our production machine (not the latest, but within 6 months).
SCSI used is LVD. Both drives and the library are on a single
SCSI chain.
I have cleaned the tape drives a couple of times. After the first
time the error showed up especially.
I will re-check the cables and terminator. Thanks for the hints.

Missing from your posting is server type and level, and tape attachment
method. Looks like Windows.
Zlatko suggested looking into the Windows Registry MAXimumSGList value.
In addition, at a more mundane level, the following from my notes:
ASC=4B+ASCQ=00 is a Data Phase Error, meaning that an error occurred
during the Data Phase of a SCSI operation; as when a SCSI target
device
receives a zero-length data frame, or too many parity errors have
occurred during the Data-In and Data-Out phases of an operation.
Could be caused by a bad (SCSI) cable or faulty SCSI termination.
I would also recommend cleaning the tape drive, to eliminate any media
processing that might contribute to this.
