If the I/O error occurs too often, you might have a vacuum pump problem.
According to IBM CE, when the vacuum pressure is too low, it can cause
tape read/write error or in our case I/O error on 3590.
Also you might already be aware of this but once the mount failed, *SM
has the tendency to put the tape to "unavailable" status. I have seen
this
happened many times. You might want to check the volume.

I have seen a couple like this before. For some reason the tape
label could
not be read. If the tape has information on it, I have no idea on
how to
get it back. If it is a scratch tape, relabel the tape and try
mounting it
again.
