We've had exactly the same problem (used nfs mounted source directory for
the upgrades).
To get around it, INDIVIDUALLY update or install each of the 4 filesets
(devices, server.gif,
server.util, server.rte), and it works fine (otherwise, lslpp -f
adsm.server.util shows no files).

And also, run "dsmserv upgradedb" afterwards.
And to load the sample scripts, run "dsmserv runfile scripts.smp".

Our previous server level was 3.1.1.5 also.
We tried to run without doing the upgradedb step, but the server would crash
whenever
server-server backup stg with maxpr=2 was attempted - after upgradedb, there
was no problem.

error experienced prior to the upgradedb, with maxpr=2 for backup stg is
below.....
ANR9999D pkthread.c (497) Run-time assertion failed
'txnP->txnlnFlight', thread 69, file tmtxn.c line 687

and from dsmserv.err....
Internal error TMTXN008 detected
