Thanks to everyone who forwarded me the duplicates with headers, I think
I've tracked down the host who has been causing the problems.  I've
removed the address from the list, so you shouldn't receive any more of
them.  If you do, please let me know.

I'm sorry this took so long to correct.  I was hosting a conference
last week and didn't have the time to devote to this problem.
