This sounds like a bug introduced around patch level 10 which was fixed at
patch level 13.  This was discussed on this forum and I believe it affects all
platforms using any SCSI library.  For AIX servers, the APAR is IC16335.

If this is your problem, the options are to upgrade or never check in a tape
unless all drives in the library are empty.
