     Ok.... you have teh ~.0.~ adsm code but installing it under AIX 4.2
     (smp)   get the ~.5~ adsm code for use on smp machines...

I just installed this patch on a test system running AIX 4.2 and had no
problems at all.  I got v2r1m0f12.IP21055.bff from index.storsys.ibm.com
yesterday.  The file is 11237376 byles long, did yours get truncated?
