Vol won't come online

Return code 12 from DISKID means that the volume is not reserved.  Before the
volume was defined to ADSM, it had to have been reserved.  The DSMINST and
DSMMDISK execs both prepare volumes for use, but it sounds like something
has happened to the volume, and it is no longer reserved.
